Longer and thicker eyelashes enjoy a spike of popularity.

Longer and thicker lashes are a common request among my female patients.  Since the FDA approval for Latisse in 2009, there have been several other brands of products that work in a similar fashion to Latisse. Although they can provide similar results, they cost less but do not require a prescription.  Care must be taken when using these over-the-counter products since they are not FDA approved and we do not know their inherent risks. Ultratherapy--New technology!!!

Check out the wonderful work my NYU colleague, Dr. Matthew White, has been working on. I was luck to work with Dr. White during my tenure at NYU, where we performed countless aesthetic and reconstructive oculo-facial plastic surgeries.  Dr. White was the lead author on the first article ever published in the medical literature on Ultherapy.  The scientific study was featured in the Archives of Facial Plastic Surgery, and demonstrated the delivery of energy to "tighten" the underlying layers of the face (termed the SMAS layer) without harming the overlying skin. This can help those that are not ready for or not interested in undergoing traditional facelift surgery.

What are injectable fillers?

More and more people are coming to see me for injectable fillers such as Juvederm and Restylane. These types of fillers are known as hyaluronic acid (HA) fillers and they improve the facial appearance by lifting and adding volume to the wrinkles and folds in the treatment area. Common treatment areas include:


1)    nasolabial folds—folds between the nose and the corner of the lips
2)    eyelids—help improved the dark circles or “bags” under eyelids
3)    cheek bones
4)    lip augmentation

There are various different types of fillers that can safely be injected to improve the appearance of the face. Injectable fillers include a variety of materials, which last for different lengths of time and have different effects and uses.  I like using HA fillers such as Juvederm and Restylane since they are safe and well tolerated in the skin.  HA fillers are injectable fillers that include hyaluronic acid, a natural complex sugar that bolsters skin elasticity, providing a smooth and supple appearance. It is biocompatible, supplementing the body's natural hyaluronic acid, which age has depleted.


HA fillers are transparent gels that are injected with a syringe into the mid- to deep dermis of your face. The dermis is the subsurface skin layer that contains connective tissue, nerve endings, sweat and oil glands, and blood vessels.


The advantage of using HA fillers such as Juvederm or Restylane is that they can offer long-lasting outcome without risks associated with permanent fillers and they can be safely repeated.
Side effects with Juvederm and Restylane are rare, but may include redness, tenderness, or pain at the injection site. If these occur, they usually subside within three days. Rarely, discoloration may occur, but this is reversible. Firmness, swelling, bumps and bruises are also rare side effects, and should subside within seven days.   As much as I try to avoid bruises, they are sometimes inevitable and patients may use concealers to cover the bruise up. A yellow-based concealer paste can often help “disguise” the underlying bruise.

How is the procedure done?
The procedure is done in the office and I usually use topical anesthetic and some ice to the area to help prevent bruising while I inject the product.   I also like to have some local anesthetic placed directly in the filler product to ensure as completely comfortable experience for my patients as possible.  The procedure usually takes anywhere form 10-30 minutes depending on the surface area of the treated areas.  

You will notice some immediate improvement in the office; but you will see the final result of the treatment a few days once the mild swelling has resolved.  You are able to resume most regular daily activities after leaving the office—your doctor should go over these with you.

How many syringes do I need and what is the cost of fillers?

This is a fairly common question and it all depends on how heavy your folds are and to what level of improvement you wish to achieve. Some end up using one syringe while others require more than 3 syringes.  The price range per syringe is anywhere from 650-800 dollars and depends on the type of product and size of syringe.

How long does it last for?

After 9-12 months, the biocompatible compound is naturally absorbed into the body.  However, some patients find improvements that last up to 1-2 years.

New publication out this month in Plastic and Reconstructive Surgery

Check out the latest publication I helped author in Plastic and Reconstructive Surgery, a medical journal, about the importance of immediately treating infections that occur near the eye and eyelid. Although rare, infections around the eyelids can occur after surgery and it is crucial for the physician to identify the early signs so that immediate treatment can be performed.

Chemical Peels: How do they work and are they really that effective??

What are they and how do they work?

There is a variety of chemical peels available on the market.  They are designed to improve the appearance of the face. Chemical peels can help to:

 

  • Reduce fine lines under the eyes and around the mouth
  • Treat wrinkles aused by sun damage, aging, and family history
  • Improve the appearance of mild scarring
  • Treat certain types of acne
  • Reduce age spots, freckles, and dark patches due to pregnancy or taking birth control pills (melasma)
  • Improve the look and feel of skin that is dull in texture and color

 

Some are available over the counter, and others, usually the more effective and potent formulations, are available by prescription from your plastic surgeon or dermatologist. 

Chemical peels work by accelerating the exfoliation of the skin process.  They can be used every so often (usually under the care of a physician) to help rejuvenate the skin. Plastic surgeons may also use chemical peels at the same time when a blepharoplasty or facelift is performed to help reduce the fine-wrinkles that surgery often doesn’t help improve.

Chemical peels are chemicals that improve and smooth the skin’s texture by removing the outer most damaged layers.  It can help improve fine wrinkles, skin blemishes and slight pigmentary changes. 

The three most common types of chemical peel categories (from most strongest peels to mildest peels) include the:

1)      Phenols (DEEP Peels)

2)      trichloroacetic acid (TCA), and

3)      alphahydroxy acids (AHAs) such as glycolic, lactic, or fruit acids (MILDEST peels)

Your doctor will use a precise formula customized to your skin to fit your best needs and wishes.  If you are interested in a mid peel to produce a smoother, brighter looking skin, mild peels such as AHAs would be suitable.  AHAs can often be applied weekly to obtain the best result as well. 

TCA peels are medium-deep peels that can improve fine wrinkles, skin blemishes and pigmentary changes.  The recovery time is usually 1-3 weeks and depends on your skin type and the TCA concentration used.  Patients should be warned that although TCA is a mild peel, some can respond with significant redness and slight discomfort for up to 2 weeks.  Darker skinned patients should be careful with higher TCA concentrations since it can change the skin color within the treated areas.   TCA peel treatment often takes 10-15 minutes and the peel depth can be adjusted depending on the desired outcomes. 

Phenol peels are the strongest of the peels and penetrate deep skin layers.   As a result, it can improve deeper facial wrinkles.  Recovery takes several weeks and can be very discomforting.  Phenol peels should be reserved for special cases under direct care of a plastic surgeon. I don’t like to perform phenol peels and would rather use laser resurfacing.

 

How to plan for a chemical peel.

Once you and your doctor have decided that you are a good candidate for a peel, you may need to be pretreated with a prescription medication to allow optimal penetration of the peel throughout the skin layers.  I like to give Retin A prior to treatment when using TCA 20% or more. Depending on the peel, you may need to arrange for someone to drive you home after the surgery as well.

 

What to expect the day of the peel.

You may be given mild sedation during the procedure, but usually not necessary with lighter peels such as AHA or TCAs.  Most peels can be performed in the office and the whole procedure can last 10-15 minutes. It’s normal to experience a slight stinging sensation for several minutes.  This is the peel working within the skin.

 

What to expect after your treatment

It is common to experience some flaking redness to the skin. Adequate lubrication to the areas peeled should be performed using aquafore. This helps prevent the skin from drying. Deeper feels can often cause significant facial swelling. You may be prescribed a daily wash to help in the healing process.

The effects of the peel may be subtle at first, but within time, you may detect a healthier shine to your skin. The effects may not be permanent, and repeated, lighter peels (maintenance peels) can help improve the general texture of your skin.
